    Nurse Practitioner, Long-term Locum, Worksite Clinic

    Shift Status: Three 7-hour shifts per week for a workplace clinic, with one-year renewable terms.   No weekends or evening shifts.

    The Nurse Practitioner works within the Wellness Centre, which houses the Wellness team and members of our nursing team and other HR and administrative staff.  The NP will ‘practice’ independently in the Clinic but there are other staff that work in the centre including a Chief Physician and a Chief Occupational Health Physician specialist.

     

    The Nurse Practitioner Role:

    The Nurse Practitioner is a valued member of our Employee Wellness team, providing care to employees in our medical walk in clinic.  The NP should expect to provide care and support to common walk in clinic complaints, including infections, coughs/colds, minor injuries and illnesses and completion of WSIB documentation, as required.  The NP will also provide referrals for diagnostic testing, specialist assessments, and prescription medication refills, as appropriate.

    The NP will be responsible to attend to those employees who have scheduled an appointment in the Wellness Centre for a health concern, although occasionally there may be an injured worker who attends for WSIB documentation.

    The NP will work within our Wellness Centre, three days weekly, with flexibility in days and hours of work; and scheduled time off can be arranged with appropriate notice.

     

    The Qualifications:

    • Master’s Degree in Nursing (MSN)
    • Completion of a recognized primary care NP-PHC program, in either the Adult stream or All Ages stream
    • Current Certificate with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary Care / Adult Care
    • Member of the Nurse Practitioners Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO) is an asset
    • Minimum of three years’ experience working as a primary care NP for adults, and Worksite Clinic experience is preferred
    • Demonstrated excellent critical thinking and problem-solving skills, organizational and interpersonal skills are needed
    • Accuro EMR experience is preferred
    • Able to commute in winter conditions
    • Basic CPR certification is required

     

    On Becoming a Nurse Consultant:

  • Registered Nurse

    Status: Part-Time for 12 month Locum plus 1 year renewals or conversion to employment.

    Position Overview:

    Working as a part of the, inter-professional care team, the Registered Nurse (RN) will deliver comprehensive, client-centered, and developmentally appropriate with a focus on mental health promotion, prevention, and culturally safe care for African, Caribbean & Black (ACB) youths and families.  The RN will prioritize the health needs of the organization’s priority population, including Newcomers and African, Caribbean, and Black (ACB) communities, engaging both internally and externally to assess population health needs and enhance well-being through targeted assessment, treatment, and prevention strategies aimed at reducing health inequities.

     

    Responsibilities:

    • Provide client-centered primary care in a non-judgmental manner, involving clients as partners in their care, recognizing the social determinants of health and associated health risks, and taking the stance of a health advocate.
    • Actively participate, seek, and provide consultation within an inter-professional model of care.
    • Provide primary and mental health care within an anti-oppressive, trauma-informed, and culturally responsive framework.
    • Perform clinical assessments and health information gathering for clients with complex needs.
    • Conduct comprehensive assessments and develop care plans for clients experiencing or at risk of mental health concerns such as depression, anxiety, trauma, and substance use.
    • Provide nursing services that address the biopsychosocial needs of clients, including those with multiple co-morbid health conditions, mental health diagnoses, and/or concurrent disorders involving substance use/abuse.
    • Prioritize client-centered care for the CHC’s priority populations, including Newcomers and ACB communities.
    • Perform intake assessment activities for new clients and provide direct access to primary care.
    • Contribute to providing more accessible, targeted services to DCHC clients through direct service, referrals, and navigation.
    • Complete timely and accurate medical records of client encounters, documentation, and third-party correspondence using the CHC’s Electronic Medical Record (EMR) system.
    • Support client safety through evidence-based practices and adherence to policies and standards, including communication and teamwork, infection prevention and control, safe medication practices, safe use of equipment and facilities, and risk reporting.
    • Contribute to activities related to the development, implementation, and evaluation of medical directives, policies and procedures, best practices, and protocols.
    • Adhere to and promote evidence-based practices and standards of best practice in keeping with the CHC clinic operational guidelines.
    • Actively participate in case conferences when required.
    • Collaborate closely with the Youth Outreach Worker, counsellors, and other providers to ensure continuity of care and holistic service delivery.
    • Support the implementation of early intervention and prevention strategies to improve youth mental wellness and community resilience.
    • Participate in chart audits, development, and implementation of Quality Improvement Plans (QIP), and accreditation activities with the Canadian Centre for Accreditation (CCA).
    • Demonstrate active participation in the CNO Quality Assurance (QA) Program to ensure the maintenance of continuing competence to practice and with the CHC’s performance management requirements.
    • Participate in the CHC agency training activities and meetings as requested.
    • Provide clinical supervision and training for nursing students when applicable.
    • Ensure client care and caseload are managed to achieve the goals of the CHC’s Multi-Sector Service Accountability Agreement (MSAA).

     

    Qualifications:

    • Bachelor of Science in Nursing (BScN) degree.
    • Current registration with the College of Nurses of Ontario (CNO).
    • Membership in the Registered Nurses Association of Ontario is an asset.
    • Phlebotomy certification required or willingness to obtain.
    • Minimum of three years of experience providing primary and/or mental health care to diverse populations.
    • Experience in a Community Health Centre (CHC) setting or other community health practice is an asset.
    • Experience working with communities and individuals facing barriers to accessing healthcare services.
    • Strong knowledge of Newcomer and ACB resources in Durham region is preferred.
    • Excellent problem-solving, organizational, and interpersonal skills, with the ability to work effectively and respectfully in a collaborative, inter-professional team environment.
    • Ability to communicate with clients in an easily understandable manner, practicing active listening to understand client needs and coaching clients as collaborative partners in their healthcare.
    • Excellent written communication skills, including report writing for a clinical setting.
    • Basic CPR certification required.
    • Valid current vulnerable sector police check required upon hire.
    • Valid Ontario Driver’s License.
    • Ability to work a pre-booked schedule with up to one evening per week in a 12 to 8pm shift.
